If the recipient is likely to stand it on a mantlepiece or desk, the design matters more than usual - it needs to work as a small piece of considered decoration as well as a card. Is this for someone formal or someone close? A birthday card for a close friend can be warmer, bolder, more personal in its design than one for a professional contact or a relative you see rarely. Both should be beautiful, but the register can differ.
